1. Open the Care360 application and click the 'Health' button on the bottom ribbon
2. Referencing the above screenshot, select the worksite you plan to visit today (this prompt is dependent on organization setting, and may not apply to some users)
3. Select all applicable symptoms, or 'I Feel Fine Today' if no symptoms apply
4. Fill out the risk assessment questions
5. Once the Risk Assessment is completed, you will be automatically redirected to your passport screen. The passport will turn green if no risk is reported. If risk factors are reported passport will remain orange. Under the passport, you will find information from your company regarding next steps.
