1. Launch the Care360 app and click 'Immunization' on the bottom ribbon
2. Click 'Request COVID-19 Accommodation' under the 'Add COVID-19 Immunization Record' option
3. Complete the request form. The below screenshot shows an example of what this form may look like, as it varies by organization. Once you have filled out the form, please await instructions/further communication from your organization
