1. To report a test result, open the Care 360 application. Click the 'More' button on the bottom ribbon, and click on 'Report an Incident'
2. Select, "Report a COVID 19 Test Result"
2. Choose how you would like to report your test result (these options will vary based on your organization's requirements)
4. Read and accept the Data Consent form
4. Complete Required Information: Test Result, Date of Test, Date of First Symptom, Test Type, and Lab or Manufacturer Name.
Additionally, select a Test Result image to Upload.
5. Please select "Test Results" to view uploaded Test Results.
