1. Go to your Care360 Dashboard

2. Click on 'Report an Incident'

3. Click on 'Report a COVID-19 Result'

4. Read disclosure and click on 'I Accept'

5. Complete sections: Test Date, Symptom Onset, Test Result, Test Type, and Lab or Manufacture Name. Then click 'Submit'

6. View case notes then click on 'Continue'

7. Answer question #1 (or #2 if applicable) then click 'Continue'

8. Review Case Notes and check mark 'Case As Complete' then click 'Save'

9. You have successfully reported your test results. You will be directed to your profile page.
